We are lucky to have a Registered Nurse on staff. In order for our nurse to best serve our school community, it is extremely important that our nurse be notified if your child goes to the hospital or Primary Doctor for the following reasons; fever, vomiting, rash, surgical procedure, etc. Please email our nurse, Clare Borah, at nurse@stclementschool.org. Also, please remember our school policies regarding illness. Your child must be fever free and free from vomiting for 24 hours prior to returning back to school.
OTC Medication Authorization Form
This includes letting the school give your child Tylenol or Advil. This for is required for OTC meds to be administered during the school days.
PK3-4, K, 6th grade and ALL new students are required to turn in the health and immunization form.
Kindergarten and all out of state students entering into St. Clement must turn in the attached document for the eye examination.
Students entering K, 2nd and 6th grade turn in the attached form for dental examination.
Students who plan to store epinephrine must turn in the attached FARE form.
Students with asthma or have a designated rescue inhaler must turn in the attached sheet/or a form stating their asthma action plan.
Prescription Medications (copy of physicians order) Form
Students storing an prescribed medications (rescue inhalers, epinephrine, diastat) must turn in the attached form along with their medical plan.
